Insurance coverage details and exclusion list

  • Group Personal Accident Insurance Scheme
  • Group Outpatient Insurance Scheme Group
  • Hospitalisation and Surgical Insurance Scheme

The coverage will be subjected to the Terms and Conditions of the insurer's Policy.

After matriculation, you will be able to use the FWD Medical Card at the panel GP clinics to enjoy the Group Outpatient Scheme. An e-version of the medical card can also be found in the FWD online portal. Students will be able to retrieve the e-card when they log into the portal, and use the e-card at the panel GP clinics. Students will also be able to submit claims for reimbursement via the FWD online portal.

  • Provides 24-hour worldwide coverage against Death due to accident.
  • Covers against Accidental Permanent Disability that results in an insured member being unable to engage in any kind of employment or occupation for the rest of his/her life due to accident except conditions stated in exclusions.
  • Covers Permanent Disablement like the loss of use of one limb due to accident, according to the Schedule of Indemnities.

Claim Procedures

  • A referral letter from your GP (for Specialist Treatment) or your Specialist (for X-ray & Diagnostic Services) or your GP/Specialist (for Physiotherapy) must be obtained.
  • All letters, original bills/receipts, hospital discharge summary (for hospitalisation, surgery or day surgery done at government/restructured hospitals) and referral letter must be submitted to Zurich Insurance Singapore within 30 days after each treatment. Claim form can be downloaded here and must be mailed to Zurich Insurance Company Ltd (Singapore Branch), 50 Raffles Place #29-01, Singapore Land Tower, Singapore 048623.
  • If the above is not possible, please give a notice of claim via email to Y2xhaW1zLnNnQHp1cmljaC5jb20= within 30 days after first treatment and follow-up with your supporting documents for claims when available.
  • Approved expenses shall be reimbursed by Zurich Insurance Singapore.

This plan pays for the clinical outpatient consultation, medical fees, outpatient diagnostic x-ray and laboratory tests when the Insured Member visits a General Practitioner under the FWD panel of clinics or polyclinics. The plan also includes consultation and treatment at a Traditional Chinese Physician, registered with the Ministry of Health's TCM Practitioners Board.

 

Claim Procedures

For Referrals to a Government/Restructured Hospital for Specialist Treatment and/or Hospitalisation/Surgery

  • If you are referred by a Panel Clinic, you are considered a Private patient.
  • To qualify as a subsidized patient, you must be a Singaporean or Singapore PR, referred by a polyclinic and without reference to a Specialist by name.
  • Where possible, students should use the FWD Medical Card (physical card or e-card) when seeking outpatient services. Should you require the manual outpatient claim form, you may download it here.

 

Panel Clinics

  • Produce your FWD medical card upon registration.
  • FWD Insurance Singapore will reimburse the Eligible Expenses, subject to the limits and Co-payment/Co-insurance shown in the Benefit Schedule.
  • You may have to top up for non-standard medicine or pay a surcharge for visits after normal clinic operating hours

 

Polyclinics and Hospital A&E Department

  • All original bills/receipts must be submitted within 30 days after each treatment to FWD Insurance Singapore.
  • If the above is not possible, please give a notice of claim via email to b3BfY2xhaW1zLnNnQGZ3ZC5jb20= within 30 days after first treatment and follow-up with your supporting documents for claims when available.
  • FWD Insurance Singapore will reimburse the Eligible Expenses, subject to the limits and Co-payment/Co-insurance shown in the Benefit Schedule.

This policy covers medical expenses incurred by a student as a result of hospitalisation and/or surgery in government/restructured hospitals.
